Ongoing support that connects your office visits, care team, medications, and follow-up around your individual health needs.

What is APCM?

Advanced Primary Care Management is a Medicare-covered approach to ongoing primary care. Your primary care clinician and care team serve as the main point of contact for your health needs, including care you receive from specialists and hospitals.

APCM helps organize care between appointments. Support is tailored to your needs and may include:

  • A personalized care plan that brings together your health conditions, treatments, and goals.
  • Chronic condition and medication support to help you understand and follow your treatment plan.
  • Care coordination with specialists, hospitals, and other health professionals.
  • Follow-up after a hospital stay to support the transition home.
  • Communication between visits through your care team and secure patient portal.

How do I participate?

Ask your provider whether APCM is appropriate for you. Participation requires your verbal or written consent; you may stop at any time. Only one provider can bill Medicare for APCM in a calendar month. An initial visit may be needed.

Your team will explain your care plan and how to contact the practice, including after-hours access for urgent needs.
