Hypertension Control Champions

The Million Hearts® Hypertension Control Champions are clinicians, practices, and/or health systems that have achieved exemplary rates of hypertension control.

These Champions are recognized for demonstrating their commitment to helping their patients with hypertension manage their blood pressure—and achieve a hypertension control rate of at least 80%.

How Champions are vetted

CDC’s published Challenge rules describe a multistep review:

  1. Data verification: Reviewers examine the reported hypertension prevalence, blood pressure control rates, and the methods used to calculate them.
  2. Medical-record validation: An independent contractor reviews a randomly selected sample of patient charts to confirm that the reported results meet the program’s criteria.
  3. Background screening: The process includes checks for serious sanctions and misconduct for individual and small-practice applicants. For large practices and health systems, reviewers assess background-check policies and their implementation.
  4. Final review: A CDC panel considers the application, background checks, and validated results before selecting Champions.
